Chronic Disease Education Series – presented by Benchmarque in partnership with Gippsland PHN.
The Chronic Disease Care Planning program is a nationally accredited program that aims to provide students with the skills and knowledge required to manage the client with chronic disease by including care planning, identifying risk factors, identifying and supporting behaviour change, engaging the client, applying health interventions and developing effective care plans.
This training is supported by funding from the Australian Government Department of Health under the Chronic Disease Support Project.
