Exploring the new MyHeart My Life program – Heart Foundation

Join the Heart Foundation and the Australian Cardiovascular Health and Rehabilitation Association (ACRA) for the opportunity to learn more about the new MyHeart MyLife patient support program.

The information session will highlight how MyHeart MyLife complements your clinical care and supports your patients to live well with heart disease.

The discussion will include:

• The evolution of MyHeart MyLife – from pilot to the new digital program.

• A live demonstration of the new program, highlighting key features, equipping you with the tools and resources to better support your patients living with heart disease.

• Practical tips to support patient education and referral to the MyHeart MyLife program.

• The latest updates and news from ACRA. The session will include one hour of live presentations and facilitated Q&A with the Heart Foundation and ACRA panel.
