Gippsland PHN has worked with Latrobe Community Health Services (LinkHC), an NDIS partner in the community on the development of this education session.

Learning Outcomes from the Session
Understand the overall role of General Practice in supporting paediatric patients and their carers to access the early childhood approach through Link Health and Community, partner of the NDIS.
- Understand the individualised pathways within the early childhood approach, including the definition of developmental delay defined under the NDIS act and eligibility criteria for the National Disability Insurance Scheme (NDIS).
- Enhance awareness of General Practitioners in relation to understanding consent, referral forms, best practice and their involvement in providing functional capacity documentation.
- To gain understanding of LinkHC/NDIS-related processes, decision-making processes, and supports for children and their families under 9 years of age.
