Topic: Dementia and end-of-life care in regional and remote Victoria: Care planning in general practice

Western Victoria, Murray and Gippsland PHNs are pleased to invite primary healthcare professionals to attend this online event.

The first in a series of webinar/s this event will cover end of life planning for people living with dementia, including the role of general practice, billing optimisation and consultations.

Discuss end of life care planning, in particular the role of General Practice in improving the end of life care journey of people living with dementia
Implement advance care planning discussions in general practice consultations, enhancing care coordination and management of palliative care needs
