DESCRIPTION:This webinar is hosted Murray PHN and Western Victoria PHN as part of a suite of webinars developed in collaboration with South Eastern Melbourne, North Western PHN, Eastern Melbourne PHN and Gippsland PHN. Presenters: Dr Elise Davey is a GP and practice principal at Kensington Hill Medical Centre, Leopold Victoria, having joined in 2017 and specialised in General Practice. Dr Elise has a strong interest in geriatric care and is a proud provider of care to patients residing in residential aged care facilities. Dr Elise is also a current board member of Hospice Foundation Geelong. Dr Paul Molina Chavez is a GP and Practice Principal at Wangaratta Medical Centre, Wangaratta Victoria. This session will provide a general overview on the palliative care journey of patients accessing general practice, and how digital health platforms can support optimisation of care. Learning outcomes: - Learn strategies to initiate conversation and provide support to patients through their palliative care journey. - Describe common challenges faced by palliative care patients and how general practices can manage them. - Implement a range of Digital Health tools to optimise palliative patient care
