MyMedicare is Australia’s new voluntary patient registration model. It will enable a blended funding model that sits alongside fee-for-service while strengthening the relationship between patients, their general practice, GP and primary care teams.
What is GPACI?
The General Practice in Aged Care Incentive supports older people living in residential aged care. This will help residents receive quality primary care services from their regular provider and practice. The General Practice in Aged Care Incentive promotes continuous, quality care.
Each quarter, providers and practices registered with MyMedicare, meeting the General Practice in Aged Care Incentive eligibility and servicing requirements, will receive an incentive payment.
Eligible practices will receive funding to manage the care for registered patients living in a Residential Aged Care Home. The payments will be quarterly, in addition to existing Medicare Benefits Scheme.
What you need to do:
Start preparing for July registration.
Gippsland PHN encourages Practices to start identifying their residents living in Aged Care through POLAR or the Practice Software, and start populating the MyMedicare registration form on behalf of your identified Residential Aged Care residents.
Doing this early preparation will reduce the pressure on GP’s, nurses and residents, as there may be residents who need a Medical Power of Attorney to sign on their behalf.
General practices can register for the GPACI and participate through MyMedicare from 1 July 2024. Your practice can also register Aged Care residents for the GPACI from 1 July.
In preparation, please make sure you:
- Register your practice with MyMedicare
Register from 1 July, before the official commencement date on 1 August 2024.
- Register your aged care resident with MyMedicare
Residents can register online, or by filling in a form and submitting it to your practice.
- Engage with the Residential Aged Care Homes you provide services to discuss the benefits of co-ordinated care and regular appointments.
GPACI supports every aged care resident to receive quality primary care services from a regular general practitioner and practice, improving continuity of care and reducing avoidable hospitalisations.
