The Commonwealth Department of Health provides funding to Primary Health Networks for the provision of psychosocial support services in each catchment area.

Commonwealth Psychosocial Support (CPS)

This service builds psychosocial functional capacity to address individual needs with a focus on building capacity and connectedness.

Gippsland PHN has established the multidisciplinary Psychosocial Support Program (PSP), which encompasses several purpose-built service streams as detailed below:

  • National Psychosocial Support Measure: providing strengths-based group and individual psychosocial support to people with severe mental health conditions, who are not more appropriately supported through the National Disability Insurance Scheme (NDIS)
  • Interface Services: support for people who are experiencing difficulty applying for the NDIS and are deemed eligible as per the criteria
  • Transition Support Services: for former clients of Partners in Recovery (PIR) who have not yet tested eligibility (or received their access decision) for NDIS and intend to do so.
  • Continuity of Support Services: providing ongoing support for clients who previously received services under the PIR and Personal Helpers and Mentors (PHaMs) programs who have been deemed ineligible for the NDIS
