Gippsland PHN is inviting general practices across the region to participate in the new Gippsland Advance Care Planning in General Practice – Quality Improvement initiative, designed to strengthen advance care planning (ACP) and palliative care delivery in primary care settings.

Community organisations, health services and local groups are encouraged to apply for a new round of grants aimed at increasing community understanding of palliative care and the importance of advance care planning.

Gippsland PHN’s latest Health Insights Paper has been released, outlining key challenges faced by the region’s primary health care sector and the actions required to meet the increasing needs of the Gippsland community.

If you, or someone you care about, is living with dementia – you’re not alone. More than 6,700 people in Gippsland are living with dementia and that number is expected to grow to over 13,000 by 2054.
