Supporting Healthy Ageing


Gippsland PHN is seeking qualified and experienced organisations to deliver early intervention activities and models of care for chronic disease management that supports healthy ageing and reduces pressure on local health services.

Current aged care services

Gippsland PHN currently commissions:

  • Mental Health in Aged Care delivered across residential facilities in Gippsland. For further information, contact Wellways or call 03 5622 4140
  • Allied Health in Residential Aged Care

Primary Health Networks have received funding to support the Australian Government’s response to the Royal Commission into Aged Care Quality and Safety.

The Royal Commission examined the complex issues being faced by senior Australians at the interface of the primary health care and aged care systems such as:

  • A lack of access to general practitioners (GPs) and allied health professionals in residential aged care facilities (RACFs);
  • Difficulties accessing after-hours services;
  • Inappropriate transfers to hospitals; and
  • Aboriginal and Torres Strait Islander people have specific needs in accessing aged care

These issues contribute to poor health outcomes for senior Australians and place increased pressure on existing  health systems.

Gippsland PHN will soon be delivering:

Two Residential Aged Care Facility programs:

  • Support Residential Aged Care facilities (RACFs) to increase availability and use of telehealth for aged care residents
  • Support Residential Aged Care facilities in the development of out of hours arrangements and action plans to help residents access urgent assessment and triage


Two community-based programs:

  • Commission early intervention initiatives that will support healthy ageing and ongoing management of chronic conditions for our senior population living in the community
  • To establish and maintain a network of care finders to provide specialist and intensive assistance to help people access aged care and connect with other relevant supports in the community

The care finder program will provide specialist assistance to senior Australians who:

  • need intensive support to understand, access and connect with aged care services. These services may include housing services, health and social support services; and/or
  • are at risk of falling through the cracks due to isolation, communication or cognitive issues, or have a history of past experiences with institutions or Government
