We are seeking qualified and experienced organisations to deliver early intervention activities and models of care for chronic disease management that supports healthy ageing and reduces pressure on local health services.
Proposals may include delivering clinical services targeted at chronic disease management or facilitating community activities that support social wellbeing and mental health for senior Australians.
Delivery of services are expected to commence from 1 January 2023 and conclude on 30 June 2024.
Activities that support the empowering of GPs and other primary health care workers through training, tools and resources which contribute to improved health and care outcomes for older people will also be considered.
