The LHIZ team has partnered with the Australian Primary Health Care Nurses Association (APNA) to support the establishment of nurse-led clinics with a prevention focus. APNA is the peak professional body for nurses working in primary health care and strives to increase awareness of the role of the primary health nurse and partner with Primary Health Networks (PHNs) to provide support and development opportunities that build capacity for nursing.
Currently APNA is seeking Expressions of Interest (EOIs) from General Practices who may be interested in establishing a nurse-led clinic as a model of care. APNA will act as the project lead and provide education to support the general practice team to adapt, develop and implement models of nurse-led care. Nurse-led clinics will have a focus on early detection and prevention of chronic disease, with a focus on preventive strategies related to chronic disease.
