Gippsland Supporting Outreach Healthcare Program

Gippsland PHN has received funding to deliver a place-based outreach model of care in East Gippsland from August 2025 through to 30 June 2027, supporting victim-survivors of family, domestic and sexual violence to access trauma-informed services. The program will reduce barriers to care for rural and high-risk communities by improving coordination across health and violence response services, strengthening the role of primary care, and supporting recovery.

Supporting the Gippsland community

Gippsland PHN was invited to participate in the Australian Government’s Supporting Outreach Healthcare for Victim/Survivors of Family, Domestic and Sexual Violence (FDSV) national pilot program. The Department of Health, Disability and Ageing (DHDA) is leading the implementation of programs that support victim-survivors of Family, Domestic and Sexual Violence (FDSV) under The National Plan to End Violence against Women and Children 2022–2032.

Gippsland PHN has been funded to commission services in the East Gippsland local government area until 30 June 2027 to develop and integrate a place-based outreach model of care to provide healthcare to support Victim-Survivors of Family, Domestic and Sexual Violence.

The intended outcomes of the program are:

  • reduce barriers to accessing healthcare for higher risk population groups.
  • reduce barriers to accessing healthcare for victim-survivors living in rural and remote communities.
  • improve the consumer journey by better integrating and coordinating the health response between the Family, Domestic and Sexual Violence sector, primary, secondary, and tertiary care services.
  • improve Family, Domestic and Sexual Violence support services’ understanding of the role of primary care in supporting victim-survivors of Family, Domestic and Sexual Violence.
  • equitably support victim-survivors during their recovery and healing journey to safely integrate back into their communities and normal activities.
