Gippsland PHN releases 2024–25 Annual Report

Gippsland PHN’s 2024–25 Annual Report highlights a year defined by courage, collaboration and innovation.

In a rapidly changing health landscape, Gippsland PHN has continued to demonstrate resilience and strong governance, working closely with partners, providers and communities to strengthen primary care and improve health outcomes across the region.

This year’s theme, Challenge to Courage, reflects how our organisation and partners have met complex challenges head-on, creating opportunities for positive change and more equitable access to care. It also captures the courage shown across Gippsland – from primary care professionals implementing reform to communities actively shaping local health solutions.

The report highlights our work to strengthen primary care, improve health outcomes and support a resilient, connected health system for the communities of Gippsland.

Some of this years’ highlights include:

  • Expanding access to care for people with the most complex needs
  • Strengthening primary care capability through digital health, data insights and quality improvement
  • Delivering trauma-informed support programs for victim-survivors of family, domestic and sexual violence
  • Improving chronic disease management and healthy ageing outcomes
  • Embedding lived experience and community voice into commissioning and service design
  • Building stronger partnerships that support place-based, community-led care
  • Enhancing system integration through improved pathways, shared care and coordinated models
  • Strengthening organisational governance, supported by strong audit results and positive stakeholder feedback

These achievements highlight the courage, collaboration and innovation demonstrated across Gippsland throughout the year. We thank all partners and providers for their ongoing commitment to improving health outcomes for our communities.
