2025 Gippsland Primary Health Awards

The Gippsland Primary Health Awards recognise local primary health organisations or teams who have demonstrated commitment to improving health outcomes for people in Gippsland.

The theme for this year’s awards is 'Challenge to Courage'.

This year’s theme, Challenge to Courage, celebrates those who turn obstacles into opportunities — challenging the status quo with innovative solutions, driving quality improvements, strengthening workplace culture, and forging partnerships that deliver better health outcomes for our communities. The 2025 Gippsland Primary Health Awards will showcase individuals and teams who have responded to challenges with courage to create meaningful change.

Award categories

Courageous Innovation

For a service that has challenged the status quo and designed a new model for care or workforce in the pursuit of improved services.

Courageous Pursuit of Quality

For a service that has shown focus and persistence in implementing a program of quality improvement to improve health outcomes.

Courageous Culture

For a service that has examined staff or patient feedback and implemented change to improve their workplace culture and support learning and capability uplift for staff.

Courageous Partnerships

Services who have looked outside their own offerings or competencies and partnered with another organisation to improve outcomes for consumers/patients.

Please outline your reasons for nominating, giving consideration to the award categories and to the evaluation criteria listed below.     • Does it provide services to the Gippsland community?

    • Is there evidence of need for the primary health service/program?
    • Does it demonstrate original ideas and a creative approach?
  • Clear objectives, impacts and outcomes
    • What were the objectives and specific results and outcomes from the primary health service/program?
    • What sort of questions were asked and how did this drive outcomes and demonstrate an innovative approach to a patient cohort or problem?
  • Consumer and partnership focus
    • Is there evidence of high patient satisfaction?
    • Does the primary health service/program enable equitable access and engagement, particularly with vulnerable groups?
    • Is there evidence of collaboration and integration with other services?
  • Workforce capability building
    • What training/support/upskilling of staff is occurring?
    • Innovative workforce models and retention strategies evident?

Courageous Partnerships

    • Does the submission describe a true partnership?
  • Responding to local needs
    • How does the service/program respond to local needs?

    • How does the partnership enhance the response to the identified need

    • What were the objectives and specific results and outcomes of the partnership?

What are the evaluation criteria?

Who can lodge a submission?

All primary health organisations or teams who have delivered a project, activity or program within the Gippsland region.

Can I nominate myself or my team for an award?

Yes, self-nominations are welcome.

Can I nominate if I have entered or won a previous award?

Yes, past applicants and winners are eligible to apply again in 2025.

Can I nominate for more than one category?

Yes, you may nominate for multiple categories or submit more than one nomination in the same category. Please note a separate nomination form is required for each entry.

Nominations close 11:59pm (AEST) Tuesday 23 September 2025.

How are nominations judged?

An evaluation panel will assess the entries according to entries against the evaluation criteria. Each application is judged on merit and the panel’s decision is final.

How will I know if I’m selected as a finalist?

Shortlisted nominees will be contacted in mid-October.

When will the winners of the 2025 Gippsland Primary Health Awards be announced?

Winners will be announced at the 2025 Gippsland Primary Health Awards presentation, held alongside Gippsland PHN’s Annual General Meeting on Wednesday 19 November 2025 at Federation University Churchill (register here). Winners will also be announced in a media release following the event.
