Join our information session for Residential Aged Care Home Staff

This session provides an overview of My Health Record, MyMedicare, and the General Practice in Aged Care Incentive (GPACI).

Attendees will gain insights into how these programs enhance care coordination, improve access to essential health information, and support better health outcomes for aged care residents.

Learning Objectives:

  • My Health Record overview
  • Accessing My Health Record
  • Security and Participation obligations for use of the My Health Record
  • Understand the MyMedicare and General Practice in Aged Care Incentive
  • Learn the benefits of these programs for residents and Aged Care Providers
  • Understand what RACHs can do to support residents

MyMedicare is a new voluntary patient registration model. It aims to formalise the relationship between patients, their general practice, general practitioner (GP) and primary care teams. Evidence shows that seeing the same GP and healthcare team regularly leads to better health outcomes.

The General Practice in Aged Care Incentive (GPACI) will make it easier for older people living in residential aged care homes to receive regular visits and care planning services from their responsible GP and practice.

Don’t miss this opportunity to enhance aged care support and patient outcomes!
