With the introduction of new care planning processes as part of the MyMedicare program, Allied Health professionals who deliver Medicare services have the opportunity to discover how to enhance their roles in the health journey for patients with chronic conditions.
Western Victoria Primary Health Network is co-hosting a Care Planning Changes for Allied Health Providers webinar on 9 December to help explain the role and responsibilities for Allied Health professionals in the new GP Chronic Condition Management Plan (GPCCMP) framework.
The session will be facilitated by healthcare leadership advisor and coach Riwka Hagen and will include a discussion from diabetes nurse educator Luke Armstrong around practical tips for implementing the new framework.
From 1 July 2025, GPCCMPs replaced GP Management Plans and Team Care Arrangements and changes to the Medicare Benefits Schedule (MBS) items for chronic disease management require patients be enrolled in MyMedicare to access management plans through the practice where they are enrolled.
GPs and prescribed medical practitioners can now refer patients with a GP chronic condition management plan to allied health services directly and patients can continue accessing multiple services required for multidisciplinary care. The requirement to consult with at least two collaborating providers has been removed.
Alongside updates on the new referral requirements, the webinar will cover how the new framework differs from the previous system; what’s changed for GPs; the impact on Allied Health practices; and the current transition arrangements. Attendees will also have chance to ask questions and discover other available resources.
This is an important chance for health professionals from a wide range of allied health fields to be informed about the new arrangements, particularly about how they can improve continuity of care for patients while maintaining business efficiency for their practice.
