Grant to address primary health care issues

Gippsland Primary Health Network (PHN) has been successful in obtaining an Australian Government grant to address primary health care service issues in the community.

Gippsland PHN applied for a grant in September 2022 under the Primary Care Rural Innovative Multidisciplinary Models (PRIMM) funding round and received $353,054 to co-design and deliver a project in both Baw Baw and Wellington Shires.

The project will build on Gippsland PHN’s previous work in designing community-led integrated models of care.
