Survey on dementia

We are helping doctors provide support and care for people with dementia. It’s not always easy to find the services, information or support you need, for you or your loved one.

You can help us by taking a quick survey. Dementia Survey (surveymonkey.com)

The anonymous survey is about your experience with your doctor and what information and support services you have found helpful. We will use this information to help improve the way GPs support people with dementia (and their carers) and to help us understand what information resources need to be developed. People who complete the survey have a chance to win a $200 EFTPOS card.

The survey closes at 11.59pm Sunday, 18 September.
