A social prescription for better health

The Gippsland Primary Health Network (Gippsland PHN) is inviting community groups and organisations to participate in Social Prescribing.

Social Prescribing is a person-centred program that looks to improve overall wellbeing in non-medical ways in conjunction with a GP or healthcare worker.

A social prescription will connect patients with a registered community group or organisation to help improve their health and wellbeing.
Interested community groups are invited to register their details in an online community directory. They can then choose to receive referrals by email. The patient can also choose to contact the community group directly in their own time.
