The Gippsland Primary Health Awards recognise local primary health organisations or teams who have demonstrated commitment to improving health outcomes for people in Gippsland.
We live in a time of rapid transformative change. Technology, demography, climate and culture are changing, providing us with opportunities to do things differently. Our award categories this year ask you to demonstrate how you’ve made changes that drove improvement, increased joy at work, provided a digital solution or increased access to care.
A multidisciplinary team in general practice or other service that has changed the way the practice approaches a patient cohort or problem.
A practice or service that can demonstrate a program that has improved workplace morale and joy in work.
A practice or service that has transformed health and wellbeing through digital solutions.
A whole of practice or service initiative that has improved access to care for a vulnerable cohort.
Please outline your reasons for nominating, giving consideration to the award categories and to the evaluation criteria. Please provide as much detail as possible when you lodge your application, including uploading supporting documents that you would like to be considered with your application.
