Gippsland PHN is inviting general practices across the region to participate in the new Gippsland Advance Care Planning in General Practice – Quality Improvement initiative, designed to strengthen advance care planning (ACP) and palliative care delivery in primary care settings.
The initiative offers up to 20 general practices a one-off grant of $5,000 (ex GST) to participate in a three-month program that aims to:
- Embed advance care planning conversations into routine health assessments
- Improve the identification of patients who may benefit from early palliative care, and
- Build whole-of-practice capability
Funded under the Australian Government’s Greater Choice for At Home Palliative Care initiative, the program includes:
- Mandatory ACP training for doctors, nurses and practice staff
- Quality improvement activities aligned to Royal Australian College of General Practitioners (RACGP) standards
- Attendance at one community-of-practice meeting
- Review of practice data and processes
- Development of processes and tools to identify and recall priority patients
Participating practices will also strengthen compliance with Practice Incentive Program Quality Improvement (PIP-QI) requirements and support RACGP accreditation and continuous improvement goals.
To be eligible, applicants must:
- Operate within Gippsland
- Be accredited against RACGP standards
- Have appropriate insurance and AHPRA‑registered clinical staff
- Commit to the full participation requirements of the initiative
