Gippsland Primary Health Network (Gippsland PHN) and the Gippsland Health Alliance (GHA) are seeking expressions of interest from Gippsland general practitioners to contribute to improvements to the current format of Electronic Medical Record (EMR) patient discharge summaries from an end user perspective.
Discharge summaries are used as a source of information for multiple audiences across the healthcare system. The Regional Discharge Summary Improvement Project has been formed following feedback from Gippsland general practitioners with the objective of improving the format of discharge summaries to ensure a well-coordinated and effective handover of a patient’s care from the hospital to their doctor.
The expression of interest process invites Gippsland general practitioners to be part of an Improvement Group alongside the GHA. Representatives can include general practice registrars.
Through a number of focus group meetings, GHA aims to develop a clear scope of work and specifications for changes to the formatting of patient discharge summaries written in the EMR, coordinate testing and implement any changes.
Participating general practitioners will be reimbursed for focus group meeting attendance.
Updates to the discharge summary are planned to be implemented by 30 June 2023.
