Do you know individuals, practices or health centres and teams, working across primary health care in Gippsland who providing significant patient-centred health services to the community?
To mark the 40th anniversary of Medicare, the Stronger Medicare Awards will honour people who have made an exceptional contribution to the health of Australians.
The awards will showcase outstanding Medicare Champions who have worked in or alongside the primary health care sector. All nominations must address at least two of the following actions and contributions made in the past 12 months:
- providing significant patient-centred health services to the community
- delivering better patient care through new and innovative ideas
- commitment to connecting patients to health care services, including in regional, rural and remote areas
- building collaborative patient care teams
- working with diverse communities or vulnerable patients to help get them healthcare
- commitment to providing quality health care in rural and regional areas
- outstanding contribution to improving primary health care to their community.
