The After-hours Toolkit has been developed to support staff and residents in Residential Aged Care Homes in the after-hours period.
It has been developed by Gippsland PHN in collaboration with other Victorian PHNs, aged care homes, Ambulance Victoria and the Virtual Emergency so that unnecessary hospital transfers are avoided and importantly residents are kept safe, ensuring they get the right care at the right time.
