Care at Home from Lifeguard™ Remote Patient Monitoring

The Care at Home from Lifeguard™ platform is a digital health solution for health providers and patients, providing the infrastructure for health providers to maintain continuity of care for people isolated in their homes

Introduction

Care at Home from Lifeguard’s remote patient monitoring solution helps healthcare providers virtually monitor “patient health status” in between visits to close information gaps in care.

Lifeguard enables the extension of care giving to those most vulnerable, significantly improves access to support systems, local clinical care teams and expert medical advice, especially in regional and remote communities. Using the remote monitoring of Patient Reported Outcome Measures (PROMs) and vital signs, patients will be monitored for early signs of deterioration by a Care Coordinator, based on site within health services.

Vital signs, PROMs and other elements related to specific conditions will be routinely recorded by the patient through an app on a smartphone device and uploaded in real time for health service monitoring.

Gippsland PHN, Gippsland General Practices and Lifeguard are working closely together to establish a truly new care model which can serve as a blueprint for the rest of healthcare to follow.

Transition information

The Gippsland PHN Remote Patient Monitoring platform Lifeguard, is becoming Care at Home from Lifeguard™.

Gippsland PHN is working with Lifeguard Health Networks, Inc. to transition all providers to Care at Home Desktop Pro, and patients from LifeguardMobile®/LifeguardRx® (Lifeguard) to the newly designed Care at Home from Lifeguard™. Care at Home from Lifeguard™ for patients works essentially the same as the Lifeguard mobile application, with a few design and performance improvements.

This transition is currently underway with the aim of having all providers and patients using the new Care at Home from Lifeguard™ before the end March.

To make the transition is easy. All accounts and data that are currently in the Lifeguard platform have been automatically synced with the Care at Home platform. This means, all patients using Lifeguard can access Care at Home from Lifeguard™ right now to access their accounts and data and continue monitoring.

General Practice Instructions

  • Download the Care at Home Desktop Pro Windows app on your laptop/desktop computer and sign in with the same credentials you use for accessing Lifeguard Desktop Pro. Tap on the button or use the link below to download the new Windows app:

Once you have downloaded Care at Home Desktop Pro, you can safely uninstall the Lifeguard Desktop Pro app.

NOTE: If your practice uses the Lifeguard Admin Portal, you can continue to use the exact same website, same as always, using your same credentials to log in to access your account and patient data. The website will be automatically rebranded to Care at Home.

Patients and or Carers Instructions

  • Patients and carers who support them in the program can simply download the new Care at Home from Lifeguard™ mobile app and sign in using the same credentials they used to access their Lifeguard account..

To continue participation in the Remote Patient Monitoring Program using Care at Home from Lifeguard™, patients can download the new mobile app from the appropriate store:

How does it work?

Practitioners in general practices, as well as providers in clinics, hospitals and bush nursing centres, can scale their ability to care for patients in between face to face visits. This is especially beneficial for patients who have chronic and complex health conditions. Clinicians can set up patients in Lifeguard via a simple email. Patients then enter their information in an online registration and install the Lifeguard app on their Android or iPhone smartphone.

Whether you are a patient or a caregiver supporting a family member or friend Lifeguard provides a “virtual safety net” making it easy for the healthcare team to monitor symptoms and possible side effects to treatments.

With Lifeguard you can:

  • Easily record symptoms and track vital measures
  • Receive alerts and guidance on how and when to contact a provider for clinical support
  • Enhanced care in between regular visits
  • Family members can be invited to support care

Gippsland PHN provides Lifeguard to assist practices to manage their chronic disease and COVID-19 positive patients remotely.

There is currently no direct cost to the practice. Lifeguard is currently licensed to eligible practices for the term of their contract with Gippsland PHN.

For Patients who want to get involved with Remote Patient Monitoring and Lifeguard?

For patients who want to get involved, they need to speak with their practices or GP first. If remote patient monitoring using Lifeguard is right for you, then you will receive an invite from your health service. It is strongly advised that patients do not download the app on their phone until prompted to do so during the invitation process.

Is the data stored safely?

Lifeguard have signed an agreement with Gippsland PHN, indicating they will abide by the Australian Privacy Principles (APPs) in the Australian Privacy Act (APA). The information entered on the App by the patient will be stored securely in Australia.
